Sending a Questionnaire to a Client
To send a questionnaire to a client, please do the following:
- Navigate to the client's profile either from All Clients or My Clients
- Navigate to the Questionnaires card
- Click on the paper airplane icon to open the Send Questionnaire Package window
- Select if you would like to send the client a new or in progress questionnaire as well as verify their contact details.
- Here you can select any questionnaires that have been started on the client's profile (will be viewable within their questionnaire card)
- Here you can select from a dropdown a new blank questionnaire to send to the client
- Nothing in the dropdown? You might need to create a questionnaire, see here for more information on how to do this
- Questionnaires added to the package through the Add In-Progress Questionnaire or Attach New Questionnaire dropdowns will be shown here
- In progress questionnaires will also include the day they were last updated
- The email where the questionnaire can be sent to
- The mobile number the questionnaire notification can be sent to
- Here a preview of the selected questionnaire will be shown
- Click here to close the window
- Click Send Questionnaire Package once you are happy with the selections
The client will now receive a notification with a link to the questionnaire package and a specific code to access it. Questionnaire's will be grouped together when sent as a package.
Resending A Questionnaire to a Client
To resend a questionnaire to a client, please do the following:
- Navigate to the client's profile either from All Clients or My Clients
- Navigate to the Questionnaires card
- Click on the expand icon
- Select Resend from the Actions column
- The notification will be resent to the same email and phone number that was originally selected. If you need to update the email/phone number you will need to send a new package as outlined here.
Questionnaire Statuses
Questionnaires will have one of the following statuses depending on how it has been actioned:
- Outstanding: The questionnaire has been sent to the client but not worked on (nothing has been saved)
- In Progress: A user has saved some results in the questionnaire but not clicked Complete Questionnaire
- Completed: A user has clicked Complete Questionnaire
Adding a Questionnaire to a Client's Profile
Alternatively, if you would like to fill out a questionnaire yourself and add it to the client's profile you can do it either:
- Directly on their profile
- During a session
How to add a questionnaire onto a Client's profile
To add a questionnaire that you complete yourself to a client's profile, take the following steps:
- Navigate to the client's profile either from All Clients or My Clients
- Navigate to the Questionnaires card
- Click on the + to open the Fill Out Questionnaire window
- Select the appropriate questionnaire from the dropdown
- Click Next
- Fill out the questionnaire and click Save As Draft if you have not finished it or Complete Questionnaire to complete it
How to add a questionnaire during a session
- When in a virtual session select Profile on the right hand side
- Scroll down to the Questionnaire card
- Either send a questionnaire to the client or add one and complete it yourself
Linking a Questionnaire to a Healthcare Service
If you would like to link a questionnaire so that it is sent out whenever a specific healthcare service is booked, please do the following:
- Navigate to Forms -> Questionnaire Manager
- Locate the questionnaire you would like to link and click Edit Healthcare Services
- From the window that opens add as many healthcare services as necessary
- Click Save when done
Viewing a Questionnaire's Response
There are two ways to view a questionnaires response from the client's profile
Directly From the Questionnaire Card
- Navigate to the client's profile either from All Clients or My Clients
- Navigate to the Questionnaires card
- Click on the questionnaire you would like to review
- A window will open that will show the questionnaire and all responses on it
From the Expanded Questionnaire View
- Navigate to the client's profile either from All Clients or My Clients
- Navigate to the Questionnaires card
- Click on the expand icon
- Click View under the Actions column for the questionnaire you would like to review
- A window will open that will show the questionnaire and all responses on it
How to Download a Questionnaire's response
Once open you are able to 'download' a questionnaires response by doing the following:
- Right click on the open questionnaire
- Select Print
- Change the printer to Save as PDF
- Alternatively, you can select a physical printer and print the responses
- Alternatively, you can select a physical printer and print the responses
- Press Save
- In the file dialog window that opens, enter the desired filename and select where it should be saved
Editing a Questionnaire's Response
To review and edit the responses of a questionnaire, please do the following:
- Navigate to the client's profile either from All Clients or My Clients
- Navigate to the Questionnaires card
- Click on the expand icon
- Select Edit from the Actions column
- A window will open where you can make any necessary edits
- When done you can either click Save as Draft to return to it later or Complete Questionnaire to finish the questionnaire
