Opening your questionnaire

To open your questionnaire please take the following steps:

1. Open your email or SMS notification and locate the questionnaire access link and questionnaire session access code. Please see the example from an email below:
   
   
   
   
   
2. Copy the 6-digit access code
3. Open the questionnaire access link
4. Input the access code
   
   
   
   
5. Press Fill Out Questionnaire
6. Fill in the appropriate information in the fields

Completing your questionnaire

Once open, to complete your questionnaire you must fill in all questions marked with a red asterisks (*). 

If your provider has sent you more then one questionnaire as part of a package they will be listed one after another.

Saving a Questionnaire for Later

To save your progress in order to finish the questionnaire later you can click Save as Draft at the end of the questionnaire you are working on.

This will mark the questionnaire as In Progress for your provider, and save your responses for later.

Finishing a Questionnaire

To complete a questionnaire, please take the following steps:

1. Access your questionnaire as outlined here
2. Fill in all fields
   • Questions marked with a red asterisks (*) are required
3. Click Complete Questionnaire at the end of the questionnaire
   • Note: If there are multiple questionnaires in the package you will be able to continue to edit responses until all questionnaires are completed
4. When all questionnaires in the package are complete you will receive a completion confirmation message advising you to close your browser
