- Go to the website https://surveyjs.io (Alternatively, you can create your own survey with any JSON editor tool and obtain the JSON script. If this is the case, skip to step 5)
- Select “Survey Creator” from the navigation bar at the top of the webpage.
- Refer to this Youtube video to learn how to create a survey in the “Survey Designer” tab of the website.

Some additional features to choose from the Toolbox on the left side of the Survey Designer page that are not mentioned in the video include:
- Signature Pad – send questionnaires/form to patients to sign
- Images – shows images within the questionnaire
- File – add files to the questionnaire
- Matrix – create a multiple choice question where the answers are presented in columns

- Once you have created your questionnaire on the “Survey Designer” tab, click on the tab “JSON Editor”. Copy the entire JSON script from the textbox.
- On the Adracare platform, go to "Settings" > “Forms” > “Questionnaire Manager”
- Click the “New Questionnaire Template” button on the top right.
- Click the “Create New Questionnaire” button.
- Add a Questionnaire Name in the “Questionnaire Name” field.
- Select any Healthcare Services which will require the questionnaire to be sent to the patient. This is what will ensure the patient receives the questionnaire in the appointment confirmation email every time an appointment is created.
- Paste your JSON script in the textbox labelled “Enter your questionnaire content here”.
- Click the “Create New Questionnaire” button.
12. From the Questionnaire Manager page, click the "Publish" button to ensure the questionnaire is sent whenever its associated healthcare service is booked.
Note 2: Once a questionnaire template is published it cannot be edited.
Note 3: If more than one questionnaire is tied to a specific healthcare service, all of the questionnaires will be packaged together and sent as one questionnaire to the patient.
