Online Intake Configuration

Through the Online Intake Configuration page you are able to select what fields appear and which are mandatory within your intake form.
This page is only available for users who are Provider Admins and can be accessed from Settings->Online Intake Configuration.
Making Changes
You are able to update the fields for the 4 parts of the intake form.
- Client Information
- Service Details
- Appointments Details
- Fees
To edit a specific page:
- Click the gear icon to the right of the page title
- Toggle fields on/off by clicking the toggle to the left of the field name

Disabled
Some fields are required by the system and cannot be toggled off
- Mark Mandatory fields by checking the mandatory checkbox
- Click Save to save your changes
Intake Pages
Client Information
The Client information page includes the following fields:
Field Name | Toggleable | Notes |

First Name | ❌ | Client's first name *is required to create their profile |
Middle Name | ✅ | Client's middle name |
Last Name | ❌ | Clients last name *is required to create their profile |
Preferred Name | ✅ | The client's preferred name which appears in all notifications and in other client facing places on the platform |
Personal Pronoun | ✅ | The client's personal pronouns which will appear on their profile for reference |
Birth Date | ✅ | The client's date of birth |
❌ | The client's email address, will be used as their contact email and if client portal is enabled their default username *is required to create their profile | |
Mobile Number | ✅ | The client's mobile number, will be used to send all system SMS notifications as well as the multi-factor authentication code to activate their client portal |
Home Number | ✅ | The client's home number, will be added to their profile for reference |
Formatted Address | ❌ | The client's address |
Civil Status | ✅ | The client's civil status, the options are:

Service Details
The Service Details area allows you to toggle on/off the following information:

Healthcare Service ID | ✅ | The desired service the client is interested in booking. This list is pulled from your Healthcare Services that have the Enable for Online Booking setting checked |
Service Languages | ✅ | The client's language |
Presenting Issue IDs | ✅ | The client's flagged presenting issues, which will include a text box where they can add additional information |
Preferred Provider Gender | ✅ | The client can indicate if they prefer a male or female provider or if they have no preference |
Appointments Details
The Appointment Details area allows you to toggle on/off the following information:

Timezone | ❌ | The client's timezone, which is used for all times listed in notifications |
Availability Selector | ✅ | When enabled, it allows the client to indicate their time preference to book an appointment |
5:00 AM - 12:00 PM | ✅ | If the availability selector is enabled, toggling this setting will show or hide the 5:00 AM - 12:00 PM time slot |
12:00 PM - 5:00 PM | ✅ | If the availability selector is enabled, toggling this setting will show or hide the 12:00 PM - 5:00 PM time slot |
5:00 PM - 9:00 PM | ✅ | If the availability selector is enabled, toggling this setting will show or hide the 5:00 PM - 9:00 PM time slot |
Location Preference | ✅ | The client's preference between meeting Online, In Person or No Preference |
Fees
The Fees area allows you to toggle on/off the following information:

Fee Type | ✅ | The client can indicate if they are interested in a Regular Fee structure or a Sliding Scale structure.
If they select Sliding Scale, they will have the ability to include their Gross Family Income range

Publishing your Changes
After saving your changes for each page a banner notification will appear to tell you that there are unpublished changes.
Click publish in the lower right to set them live.
