Adracare’s customizable intake workflow helps your team manage new client requests efficiently, ensuring the right provider is matched with the right client from the very beginning.
With flexible collectible information and built-in assignment tools, the intake feature simplifies how you gather and process essential information — before an appointment is even booked.
đź§ How the Intake Flow Works
- 📝 Client completes the intake form through your public intake link
- 🧑‍💼 The organization reviews the submission and assigns it to a specific provider
- 🙋‍♂️ Providers can request intakes for themselves
- These requests are reviewed and either accepted or declined by an admin
- 🙋‍♂️ Providers can request intakes for themselves
- âś… Provider accepts or rejects the intake
- If accepted, the provider can reach out and book with the client
- If rejected, the intake is returned to the waitlist for reassignment
🛠️ Customize Your Intake Form
The intake form is customizable and organized across four pages. You decide what which fields are mandatory and which are hidden.*select fields are required by the system
👤 Client Information
Collect client demographic information such as:
- Name, including their preferred name
- Email address
- Phone number
- Date of birth
- Personal pronouns
- Civil and employment status
🏥 Service Details
Collect details on what service the client is interested, including:
- Preferred service language
- Presenting issues
- Preferred provider gender
đź•’ Appointment Details
Find out when the client is available and if they have a preference on in person or virtual appointments.
đź’° Fees
On the Fee's page, you can include a short summary on your fee structure as well as allow the client to indicate if they are interested in a sliding scale or regular fee structure.
📬 Status Updates & Notifications
Once submitted, clients will automatically receive a confirmation notification. Afterwards, your team can move them from New to Booked or add them to the Waitlist until someone becomes available.
