Welcome to Adracare Support

Once created, questionnaires can be linked to specific healthcare services so they are sent out each time that service is booked.

1. Go to the Questionnaire Manager page (Settings -> Forms -> Questionnaire Manager)
2. Locate the questionnaire you would like to have linked to specific healthcare services
3. Click Edit Healthcare Services

   

4. From the window that opens add as many healthcare services as necessary
   
5. Click Save when done

   

When multiple questionnaires are linked to the same healthcare service they will be sent together in a package instead of individually.

On the clients profile these questionnaires will appear with the service name followed by 'questionnaire' instead of solely the questionnaire title. In the following example, the questionnaire is the PHQ-9 and when sent individually it appears as PHQ-9 Questionnaire but when sent due to being linked to a service it appears as Assessment 10 Questionnaire.

Additionally, they will be accessible from the appointment details page under Questionnaires
