Welcome to Adracare Support

Questionnaires are customizable forms that you can send directly to clients, or complete yourself. Some potential uses of questionnaires are to create:

• Consent forms
• Intake forms
• Assessments

Questionnaire creation is completed using a third party website called SurveyJS which allows users to create interactive surveys. The content of this survey is then brought into Adracare and turned into a questionnaire.
The entire process looks like this:

1. Create a Survey on SurveyJS Using their Drag and Drop Tool
2. Import it to Adracare
3. Preview and Publish the Questionnaire

Some useful fields that can be added to a questionnaire are:

• Single-Line Input: Good for collecting small amounts of text, such as a name, birthdate, or phone number
• Long Text: Collects a large amounto of text, good for free form questions such as 'Tell us more about what brings you to us'
• Checkbox: Good for questions with multiple possible answers, such as 'Check all conditions that apply'
• Radio Button: Good for questions with a single answer, such as yes or no questions
• Dropdown: Good for when you want the user to select from a specific list but prefer the look of a dropdown to a radio button
• HTML:Good for creating large blocks of text
• Signature Pad: Allows for the capturing of a user signature

Creating the Survey on SurveyJS

SurveyJS is a third party tool that allows for the creation of interactive surveys which Adracare uses to create our questionnaires

An account is not required to use SurveyJS and the first time you use their free survey designer they will provide a tour of its features. They also have a helpful tutorial videon their Youtube here.

Please note that SurveyJS is not affialiated with Adracare and any issues regarding their website should be directed to their support team here

1. Navigate to the SurveyJS website
2. Select Create Survey from the navigation bar at the top of the webpage.

Importing the Questionnaire to Adracare

After finishing your survey on SurveyJS you can bring it into Adracare and turn it into a questionnaire.

To do so, please take the following steps

While still on SurveyJS

1. Navigate from the Designer tab to the JSON Editor tab by clicking on JSON Editor

   

2. Copy all of the JSON text from the textbox

Navigate to Adracare

1. Go to the Questionnaire Manager page (Settings -> Forms -> Questionnaire Manager)
2. Click New Questionnaire Template on the right.

   

3. Click Create New Questionnaire

   

4. Add the appropriate Questionnaire Name

   

5. Select any Healthcare Services where the questionnaire needs to be sent each time it is booked. For more information please see here

   

6. Paste your JSON script in the textbox labelled “Enter your questionnaire content here”.

   

7. Click Create New Questionnaire

   

Previewing and Publishing your Questionnaire

Once created the questionnaire will appear on the list of questionnaires but will have a status of 'draft'. This means that it cannot be sent to clients or manually created however the content of the questionnaire can still be updated.

Previewing the Questionnaire

To preview a questionnaire to ensure that the content is correct, and any conditional logic is working please do the following:

1. From the Questionnaire Manager page (Settings -> Forms -> Questionnaire Manager)
2. Locate the questionnaire you would like to preview
3. Click Edit under the actions column

   

4. Click the Preview button on the right side

   

5. Review the questionnaire
6. Close the preview when happy with your review

Publishing the Questionnaire

While a questionnaire is in the draft status it can still be edited, once published the content cannot be edited so ensure that you are happy with it before publishing.

1. From the Questionnaire Manager page (Settings -> Forms -> Questionnaire Manager)
2. Locate the questionnaire you would like to publish
3. Click Publish under the actions column

   

4. Click Yes in the confirmation window
